Le locomotive del Gruppo 691 delle Ferrovie delle Stato derivano dal riadattamento delle precedenti locomotive del Gruppo 690, mediante l'applicazione dell'ampio forno del Gruppo 746 insieme al carrello bissel posteriore e ad un nuovo tipo di surriscaldatore. In seguito a questa trasformazione, queste locomotive riuscirono ad aumentare la potenza sviluppata da 1400 a 1750 cavalli.
