This is an inrō, a carrying case used for items such as medicine. It is comprised of four sections, and the case is made of wood coated with black lacquer. The front has maki-e (gold and silver lacquer) with a pattern of autumn foliage, and the back shows a spider's web made with mother-of-pearl. The netsuke (string clasp to keep the inrō close to the waist) is carved wood and designed in the shape of a demon washing the back of a skeleton. It was made in the 19th century.
