In 1922, the artist Joan Baptista Adsuara Ramos designed this medal commemorating the 400th anniversary of Magellan and Elcano's circumnavigation of the world. The front depicts the full-body figure of Juan Sebastián Elcano, in a landscape of hills and sea. It is a copy of a portrait painted by Ignacio Zuloaga in 1921, also to commemorate the 400th anniversary. The dates 1522–1922 also appear on the front. On the back, there is a profile image of the Victoria with the inscription 400TH ANNIVERSARY OF THE CIRCUMNAVIGATION OF THE WORLD, and the exergue bears the signature of Juan Sebastián Elcano.
