The certificate was issued by the 508th Parachute Infantry Regiment of the U.S. Army and signed by Colonel Roy E. Lindquist (1907–1986). Angress (1920–2010) was certified to have successfully completed parachute training and—although it was by no means true—to have made the necessary number of practice jumps.
