The title on the cover reads “Rafał Wojaczek, Psychiatric Clinic of the Medical University in Wrocław, I, 7–22 V 1965”. In addition to notes from his observation stay in hospital, where the poet was admitted after a suicide attempt, the notebook contains a few drafts of poems and notes from June and September 1965. The diary was published posthumously in “Utwory zebrane” [“Collected Works”] (1976).
The text is a record of hospital daily life and inner experiences, and testifies to a conscious assumption of the role of the “cursed poet” (poète maudit). It notes the medical diagnosis of “personalis inadequate” (inadequate personality). The manuscript is part of Rafał Wojaczek’s collection.
