Punishment Order
The Office of the District Attorney accuses you,
of having continually neglected to use, as a Jew belonging to the Reich, the additional first name, Sara, in judicial and business correspondence, in Berlin, in the non statute-barred past and up to the present time.
– Misdemeanor – in accordance with Paragraph § 13 of the law of 5.1.38 on the change in family names and first names, Reich Law Gazette I, p. 9 and the 2nd executive order of 17.8.38 (§2 and 4)
It cites in evidence: Your confession
The court deems your penalty to consist in a fine, the sum of 42—forty-two—Reichsmark and, should it prove impossible to exact this payment, in a prison sentence of 14—fourteen—days.
In addition, you will bear the costs of the court proceedings.
You may lodge an appeal against this punishment order with the undersigned court within one week, either in writing or by making a statement at the office. Otherwise the punishment order will come into effect upon expiry of this period.
The fine of 42 RM and the costs calculated below of 2 RM 50 Rpf, the total sum of 44 RM 50 Rpf, are to be paid within one week of the onset of enforceability.
The sum may be paid:
1. By sending court costs stamps to the office at the above address or
2. By payment onto the postal check account of the – court cashier – using the payment slip here enclosed
3. By bank transfer onto one of the accounts detailed below
4. By cash payment to the court cashier,

Re. 1: Court costs stamps can be purchased in any court. The stamps are to be attached to the reverse side of the payment card here enclosed and submitted, in an envelope, to the office at the above address and, expediently in the case of larger sums, in the form of a registered letter or an insured letter.
Re. 3: In the case of bank transfer, the reference number above and the legal case must be noted on the slip, or specifically communicated to the court cashier.
Re. 4: Cash payment may be effected solely in the office of the—court cashier—court cashier’s agency—.
All transmittals by post must be sufficiently franked.
If the term of payment is not observed, forcible recovery proceeds without prior warning.
