This pre-printed unused certificate is for the appointment of a pastor of the African Methodist Episcopal Church. The form has text printed in black ink and dotted lines to fill in the name of the person, their place of appointment, the date, and the signature of the presiding bishop. A dark blue seal is printed in the lower left corner. The text and the seal are surrounded by a decorative printed border. The reverse is blank.
