Bernabò Visconti (1323-1385), lord of Milan, architect of the great future Visconti state inherited by his nephew Gian Galeazzo, is remembered among the benefactors because, about one hundred years before the foundation of the Ospedale Maggiore, he had donated large possessions to the Milanese hospitals and land in Bertonico (Lodi). This portrait was commissioned by the Hospital Council in 1880 from Agostino Caironi, professor of the Chair of Drawing and Figure at the Brera Academy, who paints in the "school" style, painting both in the setting and in the characterization of the character.
