Lorenzo Brera (1846-1912), was a doctor for forty years at the Ospedale Maggiore. He was the brother of Monsignor Carlo, another benefactor of the hospital. In his wil, hel appoints his sisters Rachel and Giuseppina, to whom he leaves the task of honoring the charitable provisions he had told them "by word of mouth". Both sisters, who died in 1927 a few months later, respect the latest wishes of the doctor, each placing in their will a legacy of 200,000 in favor of Ca 'Granda. The portrait of Rachele and Carlo appear among those of the benefactors, while Giuseppina renounces it asking that her brother Lorenzo's be painted instead of hers. The commission is entrusted to Giuseppe Mascarini, author of another portrait of the Quadreria Ospedaliera.
