From the margin of the page, Saint John turns to see people kneeling before the beast who stands regally before them. In the center, the creature with "horns like a lamb" faces left to slay with a huge scimitar those who will not worship the beast. The illuminator repeated the figure of this creature facing right, where it places its sign, the number 666, on the foreheads of those who remain after the slaying, that is, on those evil people who adore Satan's creation.

The dark sword shape seen between the creature and the people is oxidized silver showing through from the other side of the page.
