EXECUTION REPORT
Warsaw, 25 May 1948. Deputy Prosecutor of the High Military Prosecutor’s Office, Major Cypryszewski Stanisław arrived at the Mokotów Prison in Warsaw at 21:30 for the carrying out of the final sentence of the Military Regional Court in Warsaw of 15 March 1948 No Sr. 103/48 approved by Decision of the High Military Court in Warsaw of 22 May 1948, No Sn. Odw. S. 924, in view of the fact that the President of the Republic of Poland refused to grant the prerogative of mercy, and pursuant to Articles 314 and 315 of the Code of Military Criminal Law, in the presence of:
1/ Prison Governor – Lt. Mońko Ryszard
2/ Physician – Lt. Dr Kazimierz Jezierski
3/ Priest – Rev. Captain Martuszewicz Wincenty, and
with the assistance of the Firing Squad Commander [SSgt.] Śmietański Piotr, after having read the sentence No Sr. 103/48 to the convict PILECKI Witold alias Jezierski Roman, son of Julian and Ludwika and after having confirmed legal validity of the sentence and announcing that the President of the Republic of Poland had not granted the prerogative of mercy, the execution of PILECKI Witold alias Jezierski Roman son of Julian and Ludwika née Osiecimska, born on 13 May 1901 in Vilnius, was carried out by firing squad.
The death of the convict PILECKI was confirmed by the physician.
